VO2 max is one of the strongest fitness markers in the research, but a lab test is only one way to track it, and your watch is not a substitute for one. Here is what the test measures, what counts as a good score, what it costs in Bangkok, and where to get it.
VO2 max testing in Bangkok has moved from sports-medicine labs into longevity programs and executive health packages, usually sold with a line about it being the single best predictor of how long you will live. The research is more interesting and more limited than that. Cardiorespiratory fitness is strongly linked to survival, and a mask test is the most accurate way to measure it. But the number is a scoreboard, not a prescription, and most of its value comes from what you do after you see it.
This guide covers what a VO2 max test measures, what the evidence says, what a good score looks like, what a test costs in Bangkok, and where to get one.

What a VO2 max test actually measures
VO2 max is the highest rate at which your body can take in and use oxygen during all-out exercise, reported in millilitres of oxygen per kilogram of body weight per minute. It reflects how well your lungs, heart, blood, and muscles work together.
In a lab, the test is done on a treadmill or a bike while you wear a mask that analyzes the gases you breathe in and out. Speed and incline rise in steps until you cannot continue. Samitivej’s description of its test is typical: about 15 minutes of exercise, roughly 30 minutes in total, then about 30 more with a doctor going through the results, which can include heart rate zones and how your body uses fat and carbohydrate at different intensities.
Two things are worth separating. A fitness or performance test is aimed at healthy people and athletes who want training zones. A clinical cardiopulmonary exercise test, or CPET, adds heart monitoring and physician interpretation and is used to investigate symptoms such as unexplained breathlessness. Some labs also offer a submaximal test, which estimates VO2 max from heart rate during an easier effort. Bangkok Hospital’s pre-marathon evaluation, for example, lists both a submaximal bike test and a direct gas analysis test. The submaximal version is less precise, and it is gentler.
Why researchers care about it
The best-known evidence comes from a Cleveland Clinic study of 122,007 adults who had treadmill tests, published in JAMA Network Open in 2018. People in the lowest fitness group had about five times the risk of death over follow-up compared with the most fit group, and the highest group had an 80 percent lower risk than the lowest. The authors reported that low fitness carried a mortality risk comparable to, or greater than, coronary artery disease, diabetes, and smoking, and that there was no upper limit of benefit.
Three caveats matter, and a good guide includes them. The study was observational, so it shows association, not proof that raising your VO2 max extends your life. Fitness was based on treadmill performance in patients referred for testing at one US center, not a mask-measured VO2 max in the general public. And fitness reflects years of habits and underlying health, so it partly tracks everything else going right.
VO2 max is a good scoreboard. It is not a prescription.
What is a good VO2 max?
A good VO2 max depends on age and sex, which is why a percentile means more than a raw number. In the FRIEND registry, a large US database of laboratory treadmill tests, the median VO2 max falls from 48.0 for men and 37.6 for women in their twenties to 24.4 and 18.3 in their seventies. Fitness drops by roughly 7 percent per decade on average, and men average roughly a quarter higher than women.
Two limits on these figures. They come mostly from the United States, and we did not find Thai-specific reference standards, so ask any lab which reference population it compares you against. And a median is not a target: the registry shows what typical tested adults achieve, not what is healthy for you. The practical takeaway is that the decline is real, steady, and slowed by training, which is the reason to track it.
Lab test, field test, or watch?
| Method | How accurate | What to know |
|---|---|---|
| Lab test with mask (direct gas analysis) | Measurement error about 5 percent | The reference standard, and the only method that measures rather than estimates |
| Submaximal test (for example a bike test) | Estimate, wider error | Safer for some people, less precise |
| Smartwatch estimate | Around 13 percent average error in recent validation studies | Estimates from heart rate, pace, and your profile |
The wearable numbers are worth knowing. A 2025 validation study of the Apple Watch against lab testing found it underestimated VO2 max by about 6 mL/kg/min on average, with a mean absolute error of roughly 13 percent, and a 2026 study of the Series 10 found almost the same. Errors of that size can move you across a fitness category. So treat a wrist estimate as a rough trend line, not a measurement, and do not make decisions from a single reading.
VO2 max test cost in Bangkok
The price of a VO2 max test in Bangkok varies by setting, and promotions move it. These were checked on provider pages in October 2026, and the examples are not a ranking.
| Provider (example) | Listed price (THB) | What to know |
|---|---|---|
| Samitivej Sukhumvit, Sport and Orthopedic Center | 6,800 (listed 7,800) | VO2 Max Physical Screening Program, price includes doctor’s fees and hospital service fees |
| Samitivej Sukhumvit, with running analysis | 8,500 (listed 10,200) | Adds a running analysis to the same program |
| Samitivej Srinakarin, Active Performance Medical Center | Same program listed | Same VO2 screening offered at a second site |
| Bumrungrad, VitalLife Scientific Wellness Center | Not published | Offers VO2 max testing through its sports and lifestyle clinic, contact for current prices |
| Bangkok Hospital | Not published | Lists submaximal and direct gas analysis options in its marathon pre-participation evaluation |
A longevity clinic’s pricing guide puts a VO2 max test at roughly 4,000 to 9,000 baht when sold on its own, which brackets the Samitivej figure. When a test is bundled into a program, the headline price covers much more than the VO2 test, so compare what is included before you compare prices.
Where to get VO2 max testing in Bangkok
VO2 max testing in Bangkok falls into three types. Hospital sports medicine centers, such as the ones above, are the most direct option if you want a standalone test with a doctor’s read. Performance and longevity clinics sell it inside broader programs with blood work and body composition. And one business in this site’s network, described at the end of this guide, includes it in a bundled workup.
Whichever you pick, ask four questions: Is it direct gas analysis or an estimate? Who supervises the test, and is your heart monitored? What reference population is your score compared against? And what do you get afterward, a printout or an explanation and a plan?
Preparing, and who should check with a doctor first
Samitivej’s guidance is to arrive rested, with at least eight hours of sleep, and to take prescribed medicines as usual. A maximal test is vigorous, so the same screening logic applies as for any high-intensity exercise: people with known heart, metabolic, or kidney disease, or symptoms such as chest discomfort or unusual breathlessness, should speak to a doctor first, and a clinical CPET may be the better choice. Our HIIT guide sets out that screening in more detail and covers training in Bangkok’s heat and haze.
What to do with the number
Retesting every few months is enough, since fitness changes over weeks to months and very frequent retests add little. The useful question is what raises it. Both interval and steadier aerobic training improve aerobic fitness, and the best plan is the one you will keep doing. VO2 max is one marker among several that matter, alongside blood pressure, blood sugar, and lipids, which is why our executive health screening guide and our biological age testing guide are worth reading alongside this one. It is also a better-supported number than the ones we flag in our telomere testing guide.
Marketing claims to distrust
- “The single strongest predictor of longevity.” The association is strong in observational data, but it does not prove that raising the number extends life.
- Lab-grade accuracy from a watch. Validation studies show errors of around 13 percent on average.
- A test sold alongside a product to “raise” it. Training raises VO2 max, not a bottle.
- Monthly retests. Real changes in fitness take weeks to months, so very frequent retests mostly show test-to-test variation.
The short version
VO2 max testing in Bangkok measures the most you can do with oxygen at all-out effort, and a mask-based lab test is the accurate way to get the number. Low fitness is strongly linked with higher mortality in a large US study, but the evidence is observational, so treat the number as a scoreboard for your training, not a verdict on your health. A standalone test at a hospital sports medicine center costs roughly 6,800 baht, while wearable estimates run about 13 percent off on average. Ask who supervises the test and what the result is compared against, check with a doctor first if you have heart, metabolic, or kidney disease or concerning symptoms, and do not retest more often than every few months.
Aspire Coaching & Physio, part of the same network as this site, runs a mask-based VO2 max test on Sukhumvit Soi 23 near Asok as one part of a Healthspan workup. The 14,900 baht workup covers over 100 markers, with the blood work read by a doctor from Orba Health, plus body composition and grip strength, and ongoing coaching is priced separately. The VO2 test itself is a mask test on the studio floor with a coach alongside, and the doctor reviews the results afterward, so the supervision questions above apply here as they do anywhere. It is not sold as a standalone VO2 test and is built for people who want coaching afterward, so if you only want a number, a hospital sports medicine center is more direct.
